Course registration form

The cut off date for registration is 3 weeks prior to class time (exceptions can be made).

If Registering for the Rejuvenating Face Massage course you will need a massage table to participate in this class. If you are travelling by means other than your vehicle (eg: bus, airplane) please contact me to be exempted from bringing a table to class.

Your course deposit of 50% of the course fee must accompany your registration to complete the registration process. The course deposit holds your spot in class so don't delay in sending it once you have completed the registration form. If paying by cheque or money order please make payable to Debbie Boehlen. If course is cancelled for any reason by the instructor a full refund will be given. The course deposit is non-refundable but may be applied toward any future course with the centre if you have to cancel.

SAVE $20.00 off your course fee by registering and paying in full for your Indian Head Massage or Rejuvenating Face Massage course at least one full month prior to the scheduled course desired.

Further Savings! Get $40.00 off your course fee by bringing a friend! For every person you bring to either the Rejuvenating Face Massage Course or the Indian Head Massage Course, who attends the course with you, and who registers and pays the full course fee at least one month prior to the scheduled course.
